Duplicate Ballot Request Form

The National Mediation Board (NMB) sent voting instructions and official mail ballots to all eligible GO Crewmembers on December 21, 2022, so you can vote in the union representation election scheduled from December 21, 2022, to February 1, 2023.  

If you have not received your voting instructions and ballot by Monday, December 26, 2022, please fill out this online form ASAP, and we will contact you and ensure you get a Duplicate Ballot Request form. IAM organizers across the system have these forms and will arrange to get one to you as quickly as possible.
