Only Recording Secretaries may fill out this form. Please fill out the form below for each of your delegates, one delegate per form. Please complete and send this form as soon as practicable; all information has to be submitted no later than July 15th, 2024.

By submitting this form, you are affirming that each delegate or alternate is a member in good standing and has been duly elected by your local lodge.
